We are seeking a skilled professional to provide high-quality Social Work services at Ryde Hospital. This is an excellent opportunity to make a meaningful difference in the lives of consumers, carers and families.
The successful candidate will be responsible for providing assessment, counselling, crisis management, case management, advocacy, intervention, appropriate referral and discharge planning services;
- Establishing effective working relationships within a multi-disciplinary team and interagency networks;
- Maintaining a commitment to professional development and quality improvement activities;
- Demonstrating skills and knowledge in the provision of service to consumers, carers and families.
To be successful in this role, you will require:
- A Social Work degree and eligibility for the Australian Association of Social Workers (AASW);
- A practical application of the AASW Code of Ethics;
- Proven ability to work independently and collaboratively;
- A commitment to participation in professional development and quality improvement activities;
- Demonstrated skills and knowledge in the provision of service to consumers, carers and families.
This is a Category A position which requires immunisation and screening for certain diseases.
