We are seeking a compassionate and empathetic Peer Practitioner to provide person-centred, recovery-oriented support. This is a permanent, part-time role (17 hours/week) at a Transitional Care Unit in St James.
Key Responsibilities- Utilize your lived/living experience of mental ill health and recovery to build connection, offer hope, and support others on their recovery journey.
- Deliver high-quality services tailored to individual needs, offering a flexible and person-centred approach.
- Provide resources and support to foster positive relationships with important people in clients' lives.
- Participate in regular training, peer co-reflection, and professional development to enhance practice.
- Support individuals to navigate services and connect with community supports that promote confidence, independence, and meaningful participation in daily life.
- Contribute to co-design and service improvement, championing the voice and leadership of people with lived experience.
The Transitional Care Unit in St James supports adults aged 18–64 to transition from hospital to community or prevent hospital admissions.
This service is delivered by a multidisciplinary team of mental health clinicians and peer workers. Together, they help clients achieve their goals, build life skills, strengthen relationships, connect with support networks, and develop the confidence and capacity to live independently.
What You Need to SucceedTo be successful in this role, you will need:
- Lived or living experience of mental health and/or alcohol and drug challenges.
- A compassionate and empathetic approach to working with clients.
- Strong communication and interpersonal skills.
- Ability to work collaboratively as part of a multidisciplinary team.
