BAYADA Home Health Care is currently seeking a skilled Occupational Therapist to perform home health visits for adult and geriatric patients. As a member of our multidisciplinary healthcare team, you will provide rehabilitative care and skilled nursing services to clients in their homes.
Key Responsibilities:
- Conduct thorough evaluations of patients' functional status and develop individualized intervention plans.
- Evaluate home environments for hazards or barriers to independent living and identify equipment needs.
- Accurately document observations, interventions, and evaluations using electronic medical records (EMR).
- Educate patients, family members, or caregivers on rehabilitative care and activities necessary for health and safety.
Requirements:
- A current MD Occupational Therapist license.
- Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
- Strong interpersonal and problem-solving skills; ability to work independently and manage time effectively.
- Solid computer skills and prior experience with EMR preferred.
