About the Role

Juniper Aged Care is seeking an Occupational Therapist to join their friendly and supportive Home and Community Care team.

This role involves working across Home Care and Transition Care Programs, supporting older adults to regain their independence after a hospital stay.

Key Responsibilities
  • Conduct client assessments to determine capabilities, goals, and support needs
  • Deliver tailored, goal-oriented therapy to support independence
  • Prescribe home modifications, aids and equipment as required
  • Provide guidance to Therapy Assistants where required
  • Communicate effectively with clients, families, and multidisciplinary teams
  • Collaborate across the organization to ensure high-quality care delivery
Requirements
  • Current registration as an Occupational Therapist with relevant authorities (essential)
  • Valid driver's license and reliable vehicle (essential)
  • Demonstrated ability to conduct functional assessments and develop person-centred therapy plans
  • Ability to work independently and as part of a multidisciplinary team
  • Strong time management and organizational skills to manage a varied caseload
