About Our Role
This is an exciting opportunity for a Registered Nurse to join our multidisciplinary team as a Nurse Care Coordinator. In this role, you will be responsible for delivering high-quality care to clients living at home, ensuring that their needs are met and they receive the best possible support.
Responsibilities
- Delivering Clinical Care and conducting comprehensive assessments and regular reviews.
- Care Planning and Coordination, advocating for clients and providing health education.
- Monitoring Health and reporting outcomes, networking and promoting our services.
What You Need
You must have:
- Registered Nurse qualifications with current AHPRA registration.
- Minimum 12 months clinical experience in acute or aged care sector.
- Current unrestricted Australian driver's licence with minimum Third-Party Damage Insurance.
- Excellent written and verbal communication skills.
- Intermediate level of computer skills.
- Sound clinical decision making and problem-solving skills.
- Demonstrated ability to work independently and as part of a team.
