Senior Social Worker

Seeking a dedicated and compassionate social worker to support patients and families in accessing vital services. This role involves conducting comprehensive assessments and providing referrals, support, and information to help clients navigate access to Government-funded aged care services and alternative support options.
Key Responsibilities
- Conduct thorough assessments of clients' needs and provide recommendations for support and services.
- Collaborate with healthcare professionals and other stakeholders to ensure seamless service delivery.
- Develop and maintain effective relationships with clients, families, and community groups.
Requirements
- A Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
- Valid NSW Drivers Licence.
- Ability to work effectively in a team environment.
