Support Coordinator Specialist

My People Solutions is seeking an experienced Specialist Support Coordinator to join their boutique Support Coordinator team in Mulgrave. As a Specialist Support Coordinator, you will have the opportunity to work with clients with complex support needs, receiving services from large multi-disciplinary teams.

Key Responsibilities:

  • Understand each client's NDIS plan and goals, align the coordination of supports and multi-disciplinary teams, and build capacity of clients.
  • Ensure reporting requirements are completed as per NDIS guidelines.
  • Answer all enquiries from clients and stakeholders in a timely manner.
  • Manage and maintain client records and information proficiently via Brevity Care.
  • Triage new client referrals, when required, and seek alternative service providers if the company is not the best fit.
  • Mentor other team members to grow and develop in their roles.
  • Use existing contacts and networks to grow the Support Coordination team's client portfolio.

Requirements:

  • 2+ years' experience in a Specialist Support Coordination NDIS role.
  • Degree level qualification in a relevant field, such as social work, psychology, allied health, community services, or mental health.
  • Experience of working with and coordinating support for people with complex disabilities, high intensity care, including mental health and psychosocial disabilities, autism & ADHD, housing, or leaving school.
  • Ability to undertake research, deliver information in an easy-to-understand format, and build key relationships with a range of diverse stakeholders.
  • Highly organised and able to manage competing priorities while delivering consistently high-quality outcomes.
  • Full competence with computer systems, apps, and other technology relevant to the role.
