**Clinical Liaison Role Overview**

The Metropolitan Referral Unit is designed to streamline access to SA Health programs that support immediate hospital avoidance and early discharge from public hospitals.

This role works closely with healthcare professionals in Southern, Central, Northern, and Women's and Children Local Health Networks to facilitate patient admissions to community SA Health programs.

Key responsibilities include:

  • Patient screening and eligibility assessment
  • Referral facilitation
  • Clinical care plan generation
  • Communication with referrers and service providers

Under supervision, the Clinical Liaison manages day-to-day referrals and stakeholder engagement, developing junior staff and maintaining accountability for practice standards and delegated tasks.
