Care Coordinator for Adults with Learning Disabilities

The Guinness Partnership is committed to providing high-quality care and support for adults with learning disabilities, physical disabilities, and complex needs. As a Support Worker in our Supported Living team, you will play a vital role in enabling customers to live their life, their way.

We are seeking highly motivated individuals to join our staff bank as Support Workers to cover available shifts as and when needed. Your primary responsibility will be to provide person-centred care and support that enables each customer to achieve their goals and live the life they choose.

Key responsibilities include:

  • Providing high-quality care and support to enable customers to achieve their goals
  • Identifying opportunities for customers to engage in leisure activities, such as bike rides, beach walks, sports, or trips into town
  • Helping customers sustain social connections by accessing the community and exploring new work placement and volunteering opportunities
  • Working on skill development to further independence
  • Supporting with everyday activities, including personal care and administering medication
  • Assisting with financial organisation, meal planning, and nutrition advice

Care or Support Worker experience is essential for this role. You should be comfortable supporting customers with personal care, which includes toileting, bathing, and dressing. Flexibility to work shifts, including early starts and late finishes, including some weekends, is also required.
