Job Title: Care Manager

We're seeking a skilled and compassionate Care Manager to join our team. As a Care Manager, you'll be responsible for delivering excellent client care, managing care plans, and overseeing the delivery of high-quality services to our clients.

Key Responsibilities:
  • Deliver exceptional client care, ensuring the needs and preferences of our clients are met.
  • Develop, monitor, and reassess care plans in consultation with clients, their families, and other healthcare professionals.
  • Manage and oversee the delivery of care services, ensuring high standards are maintained.
  • Work collaboratively with our nursing, regional, and other management teams to develop and shape our service offerings.
  • Monitor budgets and ensure clients understand service fees.
  • Manage client service delivery across all levels, including government-funded CHSP and homecare packages programs.
  • Engage with internal teams to facilitate client onboarding and lead to conversion processes.
Requirements:
  • Certificate III in Individual Support or equivalent.
  • Experience in aged care, with knowledge of Home Care Packages, Commonwealth Home Support Program, and Aged Care Quality Standards.
  • Current First Aid Certificate (HLTAID003/4) and current CPR certificate (HLTAID001) or willingness to obtain.
  • The right to work in Australia.
  • A current driver's license.
  • National Police Clearance or willingness to obtain.
