Delivering High-Quality Clinical Care in Northern Sydney

Our organization is seeking an experienced Clinical Partner to join our team in delivering high-quality, person-centered clinical care that supports clients to live well in their own home.

The successful candidate will work collaboratively with the care management team to ensure the best outcomes for clients in the community setting. The ideal candidate will possess excellent communication and collaboration skills, with a commitment to advocacy and person-centred care.

We are looking for someone who is passionate about delivering high-quality, person-centered care, with empathy, patience, and a commitment to professional development. If you have a nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General), this could be the opportunity for you.

The ideal candidate will have experience in client-focused service delivery in aged and community/home care settings, knowledge of the Aged and Home Care Reforms, and Aged Care Quality Standards. They will also be able to identify and implement innovations in care delivery and service improvement.

In order to excel in this role, the successful candidate will need to have strong analytical skills, with the ability to identify areas of improvement and develop strategies for change. They will also need to be highly organized and able to prioritize tasks effectively.

Key responsibilities will include:
