In this role, you will be responsible for all aspects of case management, including assessment, planning, and implementation of interventions to support young people and their families.

Key Responsibilities:

* Provide case management services to young people and their families, including assessment, planning, and implementation of interventions
* Develop and maintain relationships with young people, their families, and other stakeholders to ensure effective case management
* Collaborate with other professionals to ensure a comprehensive and coordinated approach to case management
* Provide support and guidance to young people and their families to help them achieve their goals and improve their well-being

Requirements:

* Tertiary qualifications in community services, social work, psychology, education, or other relevant field
* Experience in case management, particularly in the child and family sector
* Strong communication and interpersonal skills
* Ability to work in a trauma-informed way and understand the needs of vulnerable children and young people
