Job Title: Integrated Care Manager

We are seeking an experienced Integrated Care Manager to lead our strategic, operational and leadership programs.

About the Role:

The Integrated Care Manager will be responsible for the development, implementation, management and evaluation of our integrated care program.

Key responsibilities include overall program management, reporting, monitoring and evaluation to improve health outcomes and patient experience of care.

Partner with a wide range of stakeholders to redesign health services across sectors to support the implementation of contemporary models of care.

Key requirements include senior leadership experience in a substantial and complex health environment, strong record of achievement at a senior level in the health care sector and comprehensive understanding and experience in primary health care delivery in rural settings.

The successful candidate will also possess excellent leadership, communication and interpersonal skills including the ability to negotiate with stakeholders to achieve organisational reform.
