Job Title: Rehabilitation Coordinator

This role plays a crucial part in leading change, inspiring recovery and shaping the future of mental health rehabilitation. A skilled and passionate professional is required to join our multidisciplinary team.

The successful candidate will be responsible for delivering high-quality evidence-based clinical care to consumers. They will provide and coordinate clinical rehabilitation services that promote recovery and reduce disability and functional decline associated with mental illness.

Key Responsibilities:
  • Deliver High-Quality Care: Provide evidence-based clinical care to consumers.
  • Coordinate Clinical Services: Coordinate clinical rehabilitation services that promote recovery and reduce disability and functional decline.
  • Maintain Intersectoral Links: Develop and maintain a network of intersectoral links and partnerships within the sector.
  • Guide Day-to-Day Practice: Guide day-to-day practice and coordination of clinicians.
  • Escalation Point: Be a point of contact for escalation with clinical and operational matters.

The successful candidate will have strong leadership skills, experience in building relationships with multiple agency partners and the ability to adapt to changing needs and priorities. They will also have a passion for recovery-oriented care and be committed to delivering exceptional service delivery.

  • Strong Skills: Strong organizational and communication skills.
  • Autonomous Work: Ability to work autonomously but collaboratively to assess and manage clients within their clinical context.
Requirements
  • Qualification: Must hold a qualification in Nursing, Psychology, Social Work or Occupational Therapy which provides eligibility for membership of the relevant professional association and/or provisional or general registration through AHPRA as relevant.
