Community Nurse Role in Hospital at Home

This is a position that requires the skills and qualifications of a dedicated nurse who is passionate about delivering high-quality patient care.

Our hospital service provides nursing care to patients in their own homes, working closely with each client, their carer, General Practitioner and other relevant healthcare providers to develop a plan that meets the client's care needs.

The ideal candidate will have excellent communication and interpersonal skills, be able to work autonomously and as part of a team, and have a commitment to providing person-centered care.

Key Responsibilities:
  • Assess and identify the needs of patients and develop individualized care plans
  • Provide high-quality patient care and education to clients and their families
  • Collaborate with multidisciplinary teams to achieve optimal health outcomes
  • Participate in continuous quality improvement initiatives
Requirements:
  • Degree-qualified Registered Nurse with current registration with AHPRA
  • Experience in community nursing or a related field
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Commitment to ongoing professional development
