The role of a Social Worker at a hospital environment is multifaceted and involves partnering with discharge planners to manage diverse case loads ranging from future planning and financial advice, life transition for chronic disease, grief and loss adjustment to illness and disability, crisis and conflict management as well as interventions for individual and social challenges.
You will have a well-rounded knowledge of best practice approaches to patient care, including up-to-date knowledge of government initiatives such as My Aged Care and NDIS. This encompasses effective communication, active listening skills, and empathy when dealing with patients and their families.
For the position of Social Worker, we are seeking an experienced professional with:
- Bachelor of Social Work or equivalent qualification
- Membership with Australian Association of Social Workers (AASW)
- Minimum 3 years previous experience in acute Social Work within a hospital environment
- Ability to manage complex and variable caseloads
- Commitment to ongoing professional development
- Involvement in quality and service improvement initiatives
The ideal candidate will be passionate about delivering high-quality patient care and possess excellent interpersonal skills. They will also demonstrate a proven ability to deliver clinical reasoning and decision-making that supports patient advocacy.
