Job Description:

Cancer Care Coordinators play a vital role in delivering high-quality, coordinated care to patients. This position involves assessing and screening patients for their clinical and supportive care needs.

Key Responsibilities:

  • Assess and screen patients for clinical and supportive care needs
  • Facilitate timely referrals to specialists and support services
  • Ensure continuity of care between healthcare settings through clear referral pathways and communication
  • Provide education and information to patients and their families
  • Facilitate delivery of cancer care consistent with evidence-based guidelines
  • Participate in service improvement activities to enhance coordination of care and outcomes
  • Liaise between patients, medical officers, and multidisciplinary healthcare teams
  • Develop patient information resources and holistic assessment tools
  • Strengthen service networks and expand service offerings

Requirements:

This role requires a Registered Nurse licence (AHPRA), a Bachelor of Nursing degree, postgraduate oncology nursing qualification or equivalent, minimum 5 years post-graduation nursing experience, and 3+ years of clinical experience in oncology nursing. The ideal candidate will have expert clinical knowledge in managing patients with haematological and solid tumour malignancies in ambulatory and outpatient settings.
