Chronic Disease Management Professional

The role of a Chronic Disease Management (CDM) professional is pivotal in preventative health care.

Key Responsibilities:
  • Developing and implementing personalized care plans to empower patients in managing their diseases.
  • Empowering patients through education and coaching, encouraging them to take ownership of their health.
  • Guiding patients on adopting healthy behaviors and providing ongoing support.
  • Ensuring seamless access to multi-disciplinary care teams for patients.
  • Conducting comprehensive health assessments to identify chronic diseases early.
  • Collaborating with General Practitioners to promote preventive healthcare practices.
  • Increasing patient engagement and adherence to care plans.
  • Maintaining accurate records and ensuring regular review of patient care plans.

For success in this role, the ideal candidate will possess excellent organizational skills, exceptional communication and customer service skills, and a genuine passion for helping others.

Requirements:
  • AHPRA registration as an Enrolled or Registered Nurse.
  • Proven experience in Chronic Disease Management and health assessments.
  • Knowledge of health assessment principles highly regarded.
