Job Title: Palliative Care Nursing Professional
We are seeking a highly skilled and compassionate nursing professional to join our team in delivering high-quality palliative care services. As a Clinical Nurse Specialist, you will play a pivotal role in ensuring patients and their carers experience a seamless journey throughout their care from the hospital to their homes with dignity and support at every stage.
Key Responsibilities- Provide advanced clinical expertise and autonomy in the provision of complex nursing care, working collaboratively within a caring and dedicated multidisciplinary team.
- Champion, educate and lead a multidisciplinary team to improve patient outcomes as part of a cohesive care plan.
- Current registration to practice as a Registered Nurse (Division 1) with relevant qualifications or at least 3 years full time equivalent experience in Palliative Care or other related fields.
- Commitment to ongoing learning and development, with a focus on staying up-to-date with best practice in palliative care.
- Demonstrated problem solving and decision making abilities, with a strong emphasis on effective communication and collaboration.
