Home Care Facilitator

Seeking a Home Care Facilitator to join our team. As a key member of our care facilitation service, you will play a vital role in empowering individuals to live independently and improve their quality of life.

  • Working collaboratively with a multidisciplinary team to ensure seamless care delivery

Required skills and qualifications:

  • Bachelor's degree or diploma in community services or equivalent experience
  • Experience working in aged care or a related field (transferable skills highly regarded)
  • Excellent time management and organizational skills
  • Valid NSW driver's license
  • Intermediate level of computer skills
