We are currently seeking experienced Registered Nurses to fill Clinical Rehabilitation Coordinators positions at HealthcareLink.

About the Opportunity

Our Home Health team is on the lookout for passionate professionals to support our SA Community Care and My Home Hospital programs. As a Clinical Rehabilitation Coordinator, you will be responsible for delivering comprehensive client-centred management plans, ensuring effective coordination of in-home services, and building strong relationships with healthcare professionals and stakeholders.

Key Responsibilities
  • Conduct initial phone assessments, care planning, and goal setting with clients
  • Coordinate in-home services to support rehabilitation and recovery
  • Monitor patient progress and update care plans as necessary
  • Liaise with healthcare professionals and specialists
  • Discharge planning, including referrals to long-term services
  • Provide telephone support, education, and advice
About You

To succeed in this role, you will need:

  • Minimum 2 years' experience as a Registered Nurse with unrestricted AHPRA registration
  • Recent experience in care coordination
  • Strong customer service orientation and relationship management skills
  • Excellent clinical reasoning skills
  • Ability to work in a fast-paced environment
  • Proficient computer skills for data entry into patient electronic medical records
Salary and Benefits

This is a part-time or full-time position (0.6 - 1.0 FTE), offering a competitive salary range of $60,000 - $80,000 per year, depending on experience.
