Clinical Care Leader

At our facility, we are seeking a skilled and compassionate Clinical Care Leader to join our Clinical Care team. This full-time role will report to the General Manager, with support and guidance from the Clinical Care Manager.
- This position is responsible for ensuring that all residents within our facility receive the highest quality of clinical care.
As a Clinical Care Leader, you will be responsible for:
- Providing clinical expertise and leadership of clinical and care service teams across the Care Home.
- Assisting the Clinical Care Manager to lead carer teams to meet all clinical needs of residents – including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
- Managing all aspects of the assessment, documentation, and submission of AN-ACC requirements within the facility.
- Supporting the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
- Coordinating assessment, care planning, and review processes within the facility.
- Supporting the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families.
- Ensuring accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records, and staff time recording).
- Inputting and overseeing into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
- Managing and documenting clinical risk within the facility and ensuring a commitment to continuous improvements within the facility.
Key qualifications for this role include:
- An innate empathy for our residents and a commitment to continually improving the quality of clinical care provided to them.
- A minimum of 1 year experience as a Clinical Care Coordinator or similar leadership position.
- Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
- Postgraduate qualifications in aged care would be advantageous but not essential.
- Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
- Excellent communication and interpersonal skills and the ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
- Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
- Valid NDIS Workers Screening Clearance (or ability to apply).
