We are seeking a motivated and compassionate Case Manager to join our team. The successful candidate will be responsible for coordinating care teams, guiding family members, and ensuring that our clients receive the highest standard of care in the comfort of their own homes.
Requirements:
- Experience: At least 3 years of experience in case management, aged care, and/or home care, typically in a related discipline.
- Knowledge: Sound knowledge of Home Care and/or Aged Care industry, including exposure to the Aged Care Quality Code of Conduct, Aged Care Standards & accreditation process.
- Skills: Strong communication skills, demonstrated ability to build rapport with internal and external stakeholders, and a client-centric approach.
- Abilities: Proven ability to work independently, make sound judgments, and prioritize tasks in a fast-paced environment.
- Computer Skills: Proficient computer and information management skills.
