Sunrise2Sunrise Pty seeks a highly skilled Support Coordinator to join our team. As a key member of our organization, you will play a vital role in ensuring Participants receive the highest level of support to achieve their goals. This challenging yet rewarding role requires strong organizational and communication skills, as well as the ability to work independently and as part of a team.
Key Responsibilities
Your primary responsibilities will include coordinating supports to complex programs, supporting individual capacity to connect with informal, mainstream, and funded supports, and ensuring quality of supports aligns with best practice and allied health partner recommendations. You will also be responsible for facilitating regular team meetings, attending care team meetings, and completing critical incident review processes.
Required Skills and Qualifications
To excel in this role, you will require 2 years+ experience in a similar position within the NDIS industry, a solid understanding of Disability Act, and experience with Psychosocial Disability Coordination. Relevant tertiary qualifications and excellent problem-solving skills are also essential. You must be able to build rapport with clients, effectively communicate with various stakeholders, and thrive in a fast-paced customer service environment.
