Deliver exceptional care and support to clients in a dynamic and challenging role as a Complex Care Occupational Therapist.

The successful candidate will be responsible for providing comprehensive assessment, intervention, and management services to promote functional recovery, self-management, and empowerment for clients with safety, functional, medical, and environmental issues impacting independence.

About the Role

This position has two key components – Occupational Therapy and Care Coordination. As a Complex Care Occupational Therapist, you will work closely with clients to identify their needs and develop personalized plans to achieve optimal health outcomes.

Key Responsibilities:
  • Conduct home assessments to identify client needs and develop strategies to address safety and functional issues.
  • Provide equipment prescriptions and recommendations to promote independence and safety.
  • Foster collaborative relationships with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Develop and implement individualized care plans that address client goals and priorities.
Requirements:
  • Bachelor of Occupational Therapy or equivalent qualification.
  • Minimum 3 years' experience in relevant acute/community settings.
  • Strong assessment and case management skills.
  • Excellent communication and collaboration skills.
  • Knowledge of community supports available for clients.
