Home and Community Care Professional

This is a permanent part-time role that offers tailored support to individuals in their homes within North Brisbane.
The successful candidate will provide assistance to people with disabilities, illnesses or mental health conditions to maintain their independence and achieve their goals.
Key responsibilities include domestic assistance, personal care, skill development and social support, community access, and promoting independence.
- Domestic assistance such as light house cleaning and meal preparation
- Personal care inclusive of showering, daily hygiene, monitoring of health and wellbeing
- Skill development and social support
- Community access to appointments, social events and shopping
- Promote independence according to individual needs and goals
- Establish a rapport and collaborate effectively with clients and their families
Requirements include an Australian Driver's Licence, reliable vehicle with minimum third-party insurance, strong communication skills, excellent organisational skills, hygiene practices and problem-solving abilities, and a current First Aid and CPR Certificate.
Our organisation is dedicated to supporting children, young people and families, people with disability, older people and people with mental illness. We also work with people who are homeless and refugees and asylum seekers.
