Leadership and Care Coordinator Opportunity

The Leadership and Care Coordinator plays a pivotal role in providing compassionate and competent care to residents while maintaining a supportive and positive work environment for the care team.

Key Responsibilities Will Be:
  • Resident Care: Ensuring the delivery of high-quality and person-centered care to residents, addressing their specific medical, physical, and emotional needs through effective leadership and care planning.
  • Staff Supervision: Managing and leading a team of healthcare professionals, including nurses, caregivers, and support staff to achieve outstanding results.
  • Care Planning: Overseeing the development and implementation of individual care plans for each resident, considering their unique health requirements and preferences.
  • Medication Management: Ensuring proper administration and management of medications for residents, adhering to prescribed schedules and protocols.
  • Family Communication: Communicating effectively with residents' families or legal guardians, providing updates on their loved one's condition and involving them in care decisions when appropriate.
Requirements
  • AHPRA registration.
  • Minimum 2 years of Nursing experience.
  • Knowledge of Aged Care accreditation standards.
  • Comprehensive knowledge and experience with ANNAC.
  • Expertise in medication administration and wound care with a commitment to ongoing clinical excellence.
  • Strong leadership, communication, and interpersonal skills.
  • An outstanding proficiency in care planning, goal setting, and evaluation documentation.
  • Capability to effectively lead and manage a team.
