Job Overview

Responsibilities
  • Develop and implement individualised care plans tailored to each client's unique needs.
  • Monitor and respond to changes in health status, including vital signs such as blood pressure, temperature, and pulse.
  • Administer treatments such as injections, wound care, and medication management with precision and care.
  • Support clients with chronic diseases, including diabetes, dementia, and continence care, by providing comprehensive care and education.
  • Deliver palliative care and support for patients with life-limiting conditions, focusing on comfort and dignity.
  • Collaborate with multidisciplinary teams, including doctors, physiotherapists, and social workers, to ensure seamless care delivery.

Requirements
  • Current registration with the relevant Nursing Board.
  • Strong knowledge of current clinical practices and standards.
  • Understanding of Aged Care Quality Standards and ability to apply them in practice.
  • Ability to work effectively in a team environment and communicate clearly with colleagues and clients.
  • Excellent verbal and written communication skills, with the ability to provide clear explanations and instructions.
  • Strong commitment to person-centred care and empathy for the needs of elderly clients.
  • Awareness of WHS requirements, particularly manual handling and infection control.
  • Knowledge of notification and reporting obligations for Registered Health Practitioners.
  • Demonstrated understanding of privacy and confidentiality principles.
