For this role, we are seeking an individual experienced in supporting complex high-care needs including challenging behaviours. Customers have access to a wide range of professional supports and full training in their care plans is provided.

Key Responsibilities Include
- Delivering a wide range of supports assisting people with disability and mental health needs as they move from hospital to home;
- Supporting customers to engage in community-based activities;
- Supporting person-centred plans that identify a customer's life goals;
- Ensuring positive behaviour support plans are followed; and
- Administrative duties, expense and mandatory reports.
- Extensive experience in supporting customers with behaviour support plans;
- Extensive experience working with challenging behaviours- essential;
- Previous experience with manual handling and medication administration;
- Significant personal care and domestic duties experience;
- Ability to manage situations as they arise with patience and maturity;
- Certificate in disability or mental health or higher;
- A Valid drivers licence and a willingness to use your private vehicle.
