We operate a transition service in Bunbury that focuses on providing support for those living with disability and requiring mental health support, as they make the transition from hospital to home. Our clients are all unique individuals with varying needs.

For this role, we are seeking an individual experienced in supporting complex high-care needs including challenging behaviours. Customers have access to a wide range of professional supports and full training in their care plans is provided.

Key Responsibilities Include
  • Delivering a wide range of supports assisting people with disability and mental health needs as they move from hospital to home;
  • Supporting customers to engage in community-based activities;
  • Supporting person-centred plans that identify a customer's life goals;
  • Ensuring positive behaviour support plans are followed; and
  • Administrative duties, expense and mandatory reports.

What You'll Need?
  • Extensive experience in supporting customers with behaviour support plans;
  • Extensive experience working with challenging behaviours- essential;
  • Previous experience with manual handling and medication administration;
  • Significant personal care and domestic duties experience;
  • Ability to manage situations as they arise with patience and maturity;
  • Certificate in disability or mental health or higher;
  • A Valid drivers licence and a willingness to use your private vehicle.
