We are seeking a skilled Clinical Case Manager to join our team at integratedliving Australia Ltd. As a Clinical Case Manager, you will play a vital role in our 8-week Short-Term Restorative Care Program, providing care management and coordination to empower older adults to thrive.
Key Responsibilities- Partner with clients to understand their needs and develop personalized plans
- Provide ongoing support and education to clients, carers, and families on wellness and reablement
- Coordinate care with multiple disciplines, including GPs, specialists, and our Allied Health team
- Facilitate group workshops and support sessions focused on promoting healthy lifestyle and self-management skills
- Work collaboratively with other health professionals to assess, plan, and implement restorative or reablement activities for clients
- Degree qualification in nursing or allied health
- Registration with relevant professional body or Australian Health Practitioner Regulation Agency (AHPRA)
- 3+ years of clinical or health management experience in Nursing or Allied Health
- Experience within a health or community aged care setting
- Ongoing commitment to Continued Professional Development and Education
- Competitive base rate plus superannuation with eligibility to salary packaging
- Ability to work from home with some travelling required for face-to-face assessments and service promotions
- Travel reimbursement payment for use of own private vehicle
