Disease Management Nurse

The position of Registered Nurse in Chronic Care Disease Management offers a unique opportunity to make a significant impact on the lives of individuals with chronic diseases. As a key member of the healthcare team, you will play a vital role in providing care coordination, case management, and support to those affected by these conditions.
Working closely with General Practitioners, practice staff, and other relevant stakeholders, you will ensure seamless coordination of services through shared care plans and collaborative approaches. This will enable optimal support for individuals with chronic disease within the framework of the Chronic Disease Management service.
Key Responsibilities:
- Provide comprehensive care coordination for patients with chronic diseases.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with healthcare professionals to ensure consistent and high-quality care.
- Support patients and their families through education, counseling, and advocacy.
Required Skills and Qualifications:
- Current registration as a Registered Nurse with the relevant health profession regulatory body.
- Postgraduate qualification in a field related to chronic disease management (desired).
- Proven experience in care coordination and case management.
- Strong communication and interpersonal skills.
