About the Role
We are seeking an experienced and skilled healthcare professional to fill a Care Coordinator role in our multi-disciplinary community team. The successful candidate will have expertise working with older adults who have complex mental health needs, and experience in care coordination for this population.
The role involves providing education and guidance to internal and external partners, including Residential Aged Care Facility staff. Key responsibilities include assessing, planning, implementing and evaluating interventions and care plans.
Candidates must meet the following criteria:
- Registered Nurse with a minimum of 5 years full-time post-graduate experience in a mental health specialty.
- Demonstrated experience working with consumers and carers presenting with complex mental health needs and/or behaviours of concern.
- Extensive experience in conducting comprehensive mental health assessments and developing, implementing and evaluating consumer-focused care plans based on evidence-based practice.
Key responsibilities of the Care Coordinator position include:
Assessment and Planning: Conduct thorough mental health assessments, identify individualized care needs, and develop comprehensive care plans that address these needs.
Collaboration and Education: Provide education and guidance to internal and external stakeholders, including Residential Aged Care Facility staff, to support effective care delivery.
Evaluation and Implementation: Continuously evaluate and refine care plans, and implement interventions to achieve optimal outcomes for individuals receiving care.
The successful candidate will be a highly motivated and organized individual who is able to work effectively in a dynamic team environment.
Candidates must possess excellent communication and interpersonal skills, as well as the ability to maintain confidentiality and adhere to strict data protection policies.
Selection CriteriaTo be considered for this role, applicants must demonstrate the following essential selection criteria:
- A registered nursing qualification and current registration to practice as a nurse in Australia or New Zealand.
- At least five years' full-time post-graduate experience in a mental health specialty.
- Demonstrated experience working with consumers and carers presenting with complex mental health needs and/or challenging behaviors.
- Experience in conducting comprehensive mental health assessments and developing, implementing and evaluating individualized care plans.
In addition to meeting the above essential criteria, applicants should also possess strong problem-solving and analytical skills, with the ability to think critically and creatively in response to complex situations.
Knowledge of relevant legislation and policies related to mental health care, such as the Mental Health Act, would be highly regarded.
