As a Support Coordinator, you will be working with individuals facing complex challenges to provide holistic support and help them achieve the best outcomes.

The role involves implementing NDIS plans, gathering evidence, and writing reports. You will work closely with participants, stakeholders, and team members to deliver flexible and creative solutions.

To succeed in this position, you will need a strong ability to work autonomously, manage time effectively, and possess excellent communication and problem-solving skills.

You should have a Diploma or Degree in Community Services, Allied Health, or a related field, as well as experience in support coordination or case management.
