We are seeking dedicated Community Care Nurses to join our care team.
As a skilled Community Care Nurse, you will provide personalized patient care in the community, ensuring a safe and healthy environment and utilizing your clinical expertise to enhance their quality of life.
About The RoleYou will assess, plan and implement patient care within your scope, taking into account physical, social and psychological requirements. You will create and update patient plans to reflect evolving needs, ensure clinical quality standards are met, and collaborate with other multi-disciplinary team members.
No two days will be the same as you spend most of your day on the road, completing scheduled community visits.
Key Responsibilities- Hold full, unrestricted AHPRA registration and unlimited work entitlements.
- Have general nursing experience, post-registration.
- Excel in communication and clinical problem-solving skills.
- Hold a valid Australian Driver's Licence and own a roadworthy insured vehicle.
- Display confidence in managing your time effectively, including an ability to work autonomously.
