About the Role

This role involves providing high-quality physiotherapy services to elderly clients in their own homes, focusing on mobility, pain management, and strengthening. As a Community Care Specialist, you will develop individualized care plans to achieve personalized functional outcomes and work closely with local community teams and stakeholders.

Key Responsibilities
  • Provide in-home physiotherapy services to assist with mobility, pain management, and strengthening
  • Develop individual care plans to achieve personalized functional outcomes
  • Falls prevention, education, and strength training work all included in position
  • Collaborate closely with the local community teams and key stakeholders
  • Incorporate a holistic approach to healthcare that incorporates healthy living activities according to community and organisational priorities
Requirements

To be successful in this role, you will need:

  • Current registration as a Physiotherapist with AHPRA
  • Bachelor's degree in physiotherapy or equivalent studies as assessed by the Australian Physiotherapy Council and approved by AHPRA for registration
  • Prior community experience highly desirable
  • Ability to provide high-quality treatment and intervention
  • Police Check (dated within the last 3 years)
  • Willingness to obtain annual flu vaccinations
  • Evidence of Covid-19 vaccinations
  • Own vehicle and NSW licence
