Community Health Professional

This is an excellent opportunity to become a part of our multidisciplinary team at Craigieburn Community Hospital.

As a skilled healthcare professional, you will be responsible for providing assessment, delivering treatment and targeted interventions, and supporting service users and their family/carers in a collaborative environment.

You will work under the guidance of senior healthcare professionals and complete all required nursing assessments in accordance with the service user's care plan and relevant health policies.

In addition to your clinical duties, you will also participate in Duty Worker functions as required by the multidisciplinary team and manage a small caseload of consumers.

Key Responsibilities
  • Provide comprehensive assessment and treatment plans for service users and their families/carers
  • Work collaboratively with other healthcare professionals to deliver accurate, timely, and effective care
  • Develop and maintain therapeutic relationships with a diverse range of individuals
  • Participate in continuous quality improvement initiatives to enhance patient outcomes
  • Manage a small caseload of consumers and provide ongoing support and monitoring
Requirements
  • Postgraduate qualification in Mental Health Nursing or equivalent
  • Working knowledge of psychotropic medications and physical health monitoring
  • Capacity to utilize de-escalation skills and respond safely to varying situations
  • Ability to communicate and problem-solve effectively with service users, their families/carers, and other healthcare professionals
