Job Title: Care Manager

At our esteemed facility, we are seeking an exceptional Care Manager to join our team. As a key member of our multidisciplinary team, you will be responsible for providing clinical leadership and ensuring effective operations throughout the facility.

This is a challenging yet rewarding role that requires strong leadership and communication skills. The successful candidate will collaborate closely with the Facility Manager and other team members to deliver high-quality care services to our residents.

  • Promotes a client-focused approach to deliver care services based on sound clinical assessments that optimize Residents' lifestyles and maximize quality of life.
  • Provides supervision and guidance to clinical and non-clinical team members to ensure high-quality care delivery based on aged care quality standards and best practices.
  • Demonstrates positive role modeling and leadership in accordance with professional standards.

The ideal candidate will have:

  • Registered Nurse qualification with minimum 5 years' experience.
  • Experience in aged care, preferably in residential care settings.
  • Clinical expertise in geriatric care management.
  • Understanding of AN-ACC funding models and quality/risk management systems.
  • High level computer literacy in Microsoft Suites and software skills.
  • Excellent verbal and written communication skills.
  • Ability to develop staff, foster teamwork, and effectively work as a team member while providing leadership.
