The Transitional Aged Care Program (TACP) is a goal-oriented, time-limited initiative that provides therapy-focused care to support older adults in living safely and independently in their own homes. The program aims to empower individuals by guiding them toward dignity and vibrant living.
As an Occupational Therapist, you will play a crucial role in bridging vulnerability and independence. You will work closely with the TACP team, Allied Health Team Leader, and Manager in a new position that offers scope for hybrid models of care delivery.
The role involves cooperation and support within a strong leadership team, addressing significant challenges in a changing aged care environment impacted by growing patient populations and health reforms.
The Aged Care Division delivers a range of clinical and non-clinical services across the Local Health District to support older people and their families in remaining supported at home.
