Job Title: Clinical Care Coordinator & Nurse Mentor

About the Role:

We are seeking a dedicated and experienced Clinical Care Coordinator & Nurse Mentor to join our team. The successful candidate will be responsible for ensuring that all residents receive the highest quality of clinical care.

Key Responsibilities:

  • Provide clinical expertise and leadership to clinical and care service teams across the facility.
  • Assist in leading carer teams to meet all clinical needs of residents – including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
  • Manage all aspects of the assessment, documentation and submission of AN-ACC requirements within the facility.
  • Support the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
  • Coordinate assessment, care planning and review processes within the facility.
  • Support the admission of new residents to facilities for optimal care assessments and initial consultations with resident and families.
  • Ensure accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records and staff time recording).
  • Input and oversight into the facility team rosters and ensuring optimal skill mix available for delivery of care to residents.
  • Manage and document clinical risk within the facility and ensure a commitment to continuous improvements within the facility.

Requirements:

  • Minimum 1 year experience as a Care Coordinator or similar leadership position.
  • Current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
  • Post graduate qualifications in aged care would be advantageous but not essential.
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.
  • An engaging style that has ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
  • A high level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
  • A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
  • Ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
  • A valid NDIS Workers Screening Clearance (or ability to apply).
