Clinical Care Coordinator and Nurse Mentor Role

This is a full-time position that requires a dedicated clinical care coordinator and nurse mentor to lead the care team in providing high-quality clinical care to residents. The role reports to the facility manager, with support from the care manager.

The Key Responsibilities

The key responsibilities of this role include:

  • Providing clinical expertise and leadership to the care team to ensure that all residents receive the highest quality of clinical care.
  • Assisting the care manager in leading the care team to meet all clinical needs of residents, including monitoring and tracking clinical indicators to achieve best practice clinical care outcomes.
  • Managing all aspects of the assessment, documentation, and submission of AN-ACC requirements within the facility.
  • Supporting the effective documentation and full clinical team user support of policies, procedures, and IT system requirements in the facility.
  • Coordinating assessment, care planning, and review processes within the facility.
  • Supporting the admission of new residents to facilities for optimal care assessments and initial consultations with residents and families.
  • Ensuring accurate information is recorded and in accordance with all organisational and legislative requirements (AN-ACC, resident records, and staff time recording).
  • Inputting and overseeing facility team rosters and ensuring an optimal skill mix is available for delivery of care to residents.
  • Managing and documenting clinical risk within the facility and ensuring a commitment to continuous improvements within the facility.
Required Skills and Qualifications

To be successful in this role, you will have:

  • A minimum of 1 year experience as a care coordinator or similar leadership position.
  • A current unrestricted Registered Nurse Div 1 with Australian Health Practitioners Regulation Agency (AHPRA).
  • Postgraduate qualifications in aged care would be advantageous but not essential.
  • Experience and extensive knowledge of Aged Care clinical assessments, care plans, AN-ACC, and accreditation compliance and documentation.
  • An engaging style that has the ability to easily influence and develop clinical leadership skills across the clinical care team in an aged care environment.
  • A high level of computing skills to ensure that documentation and reporting requirements can be maintained in accordance with organisational and legislative requirements.
  • A confident team player with excellent communication and interpersonal skills that has a passion for leading clinical outcomes.
  • The ability to manage multiple stakeholders and have a balanced and level-headed approach to emergency and stressful situations that arise.
  • A valid NDIS Workers Screening Clearance (or ability to apply via NDIS).
