The Home-Based Care Clinician will lead the development and implementation of clinical programs and services that promote high-quality, patient-centered care to adults aged 65+ in their homes or residential settings. This role requires strong clinical knowledge and leadership skills.

About the Role
This position involves working collaboratively with multidisciplinary teams to develop and implement effective care plans. The successful candidate will have strong communication and interpersonal skills, as well as the ability to work autonomously and make sound clinical decisions.

Responsibilities
