West End, Australia NSW Government Full time
About the Role

This position focuses on supporting young people with complex or high needs, who may be difficult to engage in effective treatment and support. The aim is to guide the future management of these individuals in primary health settings, provide evidence-based interventions, or facilitate referral to specialist settings when required.

The role involves working with young people who may exhibit behaviour indicative of a developing mental health problem, such as prodromal symptoms recognized by parents, teachers, or other service providers. These individuals may require timely access to appropriate and targeted clinical care to avoid unnecessary hospitalizations.

The identified outcomes for this position include:

  • Successful therapeutic engagement with young people experiencing or at risk of severe mental illness, who may be considered very hard to engage;
  • Better experience of care through integrated and coordinated service models, particularly with headspace centres, child and adolescent mental health services, hospitals, and schools;
  • Avoidance of unnecessary hospitalizations through timely access to appropriate and targeted clinical care;
  • Active use of a multiagency care plan for young people with or at risk of severe and complex mental illness to link providers across multiple services involved in their care;
  • Improved support for families and carers of young people experiencing severe mental illness.
