**Disability Support Role Opportunity**

We are seeking a dedicated and experienced support worker to join our team in providing high-quality care to individuals with physical and mental health disabilities.

The successful candidate will be responsible for supporting daily living skills, assisting with social and recreational activities, implementing and monitoring practices that promote a healthier lifestyle, and empowering participants to achieve their goals.

The role requires working with participants who have both low and complex needs, including those who require assistance with medication prompts and may exhibit complex behaviors.

To be successful in this role, you will need to demonstrate experience and professionalism, as well as strong communication and interpersonal skills.

You will work collaboratively with staff, management, participants, and their families to achieve positive outcomes and ensure a smooth service delivery flow.

**Key Responsibilities:**

- Provide personal care and support to participants with daily living skills

- Assist with social and recreational activities to promote a healthier lifestyle

- Implement and monitor practices that promote independence and empowerment

- Collaborate with staff and management to achieve positive outcomes

**Requirements:**

- 1 year of experience in disability personal care (preferable)

- First Aid Certification (preferable)

- Working with Children Check (preferable)

- National Police Certificate (preferable)

- Driver Licence (required)

**Benefits:**

**Schedule:**

- Afternoon shift

- Day shift

- Morning shift

**Additional Responsibilities Include:**

- Building rapport and collaborating effectively with staff/management, participants, their family, and friends

- Assisting with liaison with participants' families and friends to explore new goals, ambitions, promote and empower participants to achieve their goals and provide a medium of solid communication

- Increasing participant skills with your active support and promoting independence by enhancing their skills and personal development

- Providing quality and professional support in line with participants plans, needs, and wants

- Assisting participants with accessing their social community to engage with their friends and social groups to promote a healthier lifestyle

- Implementing strategies and programs to develop/enhance participant skills, identify areas of increased support and opportunities for services to help achieve their goals
