The Homes First program is dedicated to providing a supportive environment for individuals and families in need of housing assistance. Our team of skilled case managers work closely with clients to identify their unique needs and develop personalized support plans that help them achieve stable, long-term housing.
This role is critical in helping clients overcome personal challenges and addressing their mental and physical health needs. By offering flexible, trauma-informed, and culturally safe case management support, we empower our clients to take control of their lives and work towards securing suitable and sustainable housing.
Key Responsibilities:
- Conduct thorough needs assessments to understand the client's situation and develop effective support plans
- Provide practical support where necessary, connecting clients with relevant services and resources
- Offer flexible, trauma-informed, and culturally safe case management support for up to three years
