Pharmacist Role

We are seeking a motivated, professional Pharmacist to join our team.

This is an opportunity to build clinical pharmacy skills and experience in a full-time position.

The role involves working across the pharmacy dispensary as well as inpatient wards of multiple campuses.

You will work closely with hospital staff and patients to deliver a consistently high level of clinical pharmacy service by providing support and information relating to medication and medication safety practices.

The main responsibilities include:

  • Compiling and documenting best possible medication history for hospital inpatients
  • Reconciliation and clinical review of prescribed medications
  • Managing supply of inpatient and discharge medications for hospital patients and some outpatients
  • Checking prepared medications prior to delivery to wards and patients
  • Preparation of discharge medication and discharge medication summaries
  • Medication education and counselling for patients and their carers
  • Working within a team of pharmacists, technicians and students providing supervision and assistance as required and contributing to improvement activities
  • Liaison with external facilities and community health care providers to ensure continuity of care for individual patients
  • Providing medication related information, advice and support to the multidisciplinary team.

The role also offers opportunities to develop clinical pharmacy skills and expertise within a range of clinical specialties including paediatrics and adolescent health, oncology and haematology, coronary care and interventional cardiology, obstetrics and gynaecology, and special care and neonatal intensive care.

To be considered for this role you must have a Bachelor of Pharmacy and APHRA registration to practice as a Pharmacist (without restrictions).

Other highly regarded attributes include prior demonstrated experience in ward-based clinical pharmacy, the flexibility to work all shifts, including evenings, weekends, public holidays and on-call, and the ability to work at all Mater Hospital locations.
