Job Title: Maternal and Child Health Professionals

We are seeking skilled professionals to provide targeted Enhanced Maternal and Child Health services to vulnerable children and their families.

About the Role:

In this role, you will work with families experiencing early parenting difficulties through family-focused and collaborative wrap-around care models. You will provide high-quality evidence-based practice with a focus on strengthening parenting capacity, improving family functioning, and promoting the health and wellbeing of vulnerable children and their families.

  • Deliver in-home support to families through targeted interventions and referrals to community resources.
  • Develop and implement individualised care plans to address the unique needs of each family.
  • Work collaboratively with other healthcare professionals to ensure seamless service delivery and coordination.
  • Foster strong relationships with families, empowering them to make informed decisions about their child's health and development.
  • Provide education and information on various aspects of parenting, including breastfeeding, early learning, nutrition, and immunisation.
Key Responsibilities:
  • Assessing the needs of families and developing targeted interventions.
  • Providing ongoing support and guidance to families.
  • Collaborating with other healthcare professionals to ensure comprehensive service delivery.
  • Maintaining accurate records and reporting outcomes.
Requirements:

To be successful in this role, you will require:

  • A Division 1 Registered Nurse (AHPRA) or equivalent qualification.
  • A Graduate Diploma, Masters, or equivalent qualification in Maternal and Child Health, Family and Child Health, or a related field.
  • Previous experience working with vulnerable families and children.
  • Strong communication and interpersonal skills.
  • A current Victorian driver's licence.
