Key Responsibilities:
- Provide comprehensive nursing care to patients with mental health conditions, including assessment, planning, implementation, and evaluation of care.
- Utilize the CIMHA system for effective case management and patient documentation.
- Conduct patient intake and assessments, using your skills to identify needs and develop care plans.
- Collaborate with multidisciplinary teams to provide comprehensive care and support.
- Educate patients and their families on mental health management and community resources.
Requirements:
- Minimum 2 years of experience in community mental health nursing.
- Knowledge of CIMHA and experience in using the system for case management and patient documentation.
- Skills in patient intake and assessment, with the ability to identify needs and develop care plans.
- Experience working in a multidisciplinary team environment.
- AHPRA Registration (must be current).
- National Police Check.
- Immunisation History.
- Additional Mandatory Trainings.
