About the Role

We are seeking a dedicated and experienced Clinical Nurse to join our team at CollarCare. As a key member of our multidisciplinary team, you will be responsible for delivering high-quality nursing care to patients in a community mental health setting.

Key Responsibilities:

  1. Provide comprehensive nursing care to patients with mental health conditions, including assessment, planning, implementation, and evaluation of care.
  2. Utilize the CIMHA system for effective case management and patient documentation.
  3. Conduct patient intake and assessments, using your skills to identify needs and develop care plans.
  4. Collaborate with multidisciplinary teams to provide comprehensive care and support.
  5. Educate patients and their families on mental health management and community resources.

Requirements:

  1. Minimum 2 years of experience in community mental health nursing.
  2. Knowledge of CIMHA and experience in using the system for case management and patient documentation.
  3. Skills in patient intake and assessment, with the ability to identify needs and develop care plans.
  4. Experience working in a multidisciplinary team environment.
  5. AHPRA Registration (must be current).
  6. National Police Check.
  7. Immunisation History.
  8. Additional Mandatory Trainings.
