About the Role
We are seeking a Wellbeing Mentor to join our team at Ermha365. As a Wellbeing Mentor, you will work collaboratively with our team to provide case management, assertive outreach, and group facilitation to support participants and their families in their own home and within their community.
Key Responsibilities:
- Provide case management and assertive outreach to support participants and their families
- Facilitate group sessions to promote independence, development, and confidence
- Develop and maintain professional relationships with participants and stakeholders
- Provide accurate and timely case notes, including risk management and recovery plans
