About the Role

We are seeking a compassionate and experienced Aged Care Assistant to join our team at Right at Home. As a key member of our care team, you will provide high-quality care and support to our clients in their own homes, helping them to maintain their independence and live empowered lives.

Key Responsibilities
  • Provide personal care and domestic support to clients, including assistance with daily living activities, meal preparation, and medication management.
  • Develop and implement personalized care plans to meet the unique needs and goals of each client.
  • Work collaboratively with clients, families, and other healthcare professionals to ensure seamless care coordination.
  • Conduct regular assessments and evaluations to monitor client progress and adjust care plans as needed.
  • Maintain accurate and detailed records of client interactions, care provided, and any concerns or issues.
Requirements
  • Certificate III or higher in Aged Care or Disability Services.
  • Valid Australian Driver's License and access to a reliable vehicle.
  • Satisfactory Australian National Police Check.
  • Current or willing to obtain a First Aid and CPR certificate.
  • Smart mobile phone.
  • Ability to work autonomously and as part of a team.
  • Excellent written and verbal communication skills.
  • High level of time management and organizational ability.
