As a Disability Support Specialist, you'll work collaboratively with individuals, families, and support teams to empower people with disabilities to achieve their goals and live fulfilling lives.
Key Responsibilities:- Conduct comprehensive assessments using evidence-based tools to identify individual needs and develop personalized plans
- Collaborate with interdisciplinary teams to provide person-centered support and planning, ensuring participants achieve optimal outcomes
- Build strong relationships with participants, families, and support teams to promote empowerment, independence, and community engagement
- Bachelor or Master's degree in Disability or related field
